Description

CMS announced disbursement of the 2024 APM Incentive Payments to eligible clinicians who were Qualifying APM Participants (QPs) based on their 2022 performance. When CMS prepared to disburse payments, we were unable to verify banking information for the QPs listed in the Excel spreadsheet in this zip file. We need your assistance in verifying banking information. This zip file includes the Excel spreadsheet and a PDF of the APM Incentive Payment Notice and 2024 Billing Collection form.
